Inmunoterapia activa con una variante estructural del ligando 1 del receptor de muerte programada 1

Resumen

Introducción: Los inhibidores de los puntos de control inmunitario originaron un cambio de paradigma en el tratamiento del cáncer. En este contexto la inmunoterapia activa abre una vía alentadora para la terapia tumoral dirigida a esta molécula.

Objetivos: Desarrollar una vacuna basada en una variante estructural del PD-L1 humano que induzca respuesta inmune específica con efectos antitumorales, y estudiar los efectores inmunológicos y su impacto en el microambiente tumoral.

Métodos: El desarrollo preclínico de PKPD-L1Vac incluyó: diseño in silico del antígeno; expresión en E. coli; caracterización fisicoquímica; evaluación de inmunogenicidad en ratones y primates no humanos; y estudio de actividad antitumoral en modelos murinos.

Resultados: Se generó un antígeno quimérico del dominio extracelular de PD-L1 humano con modificaciones estructurales que reducen su unión a PD-1 y CD80. La vacuna indujo anticuerpos IgG específicos anti-PD-L1 e inmunidad mediada por linfocitos T en ratones y primates no humanos. Mostró actividad antitumoral en modelos CT-26 y B16-F10. Además, aumentó los linfocitos infiltrantes en tumores CT-26 y disminuyó la proporción de células T anérgicas CD3+CD8+PD1+.

Conclusiones: Estos resultados constituyen la prueba de concepto de una vacuna novedosa basada en una variante modificada de PD-L1 que induce una potente respuesta inmune con actividad antitumoral efectiva en múltiples modelos singénicos de ratón, así como la ruptura de tolerancia inmunológica al PD-L1 humano en un modelo filogenéticamente cercano.
